mnz incidents reports The summaries in the following text are taken from the monthly reports published by Maritime New Zealand. The incidents in the summary are reported by the person advising Maritime New Zealand of the accident or incident in question and may not necessarily be in accord with the findings of Maritime New Zealand following subsequent investigation. Injury only… Injury only… April 21 Mount Maunganui 17.26m NZ non-passenger (SSM) trawler A crewmember attempted to lift a bag containing water and swing it onto the deck. In the process he lost his balance and struck his chest on the secured tow hook resulting in suspected cracked ribs. April 28 Hobson Basin Wharf 18.10m NZ passenger (SSM) In less exciting reportage, a crewmember on a tourism activity vessel moved aft from foredeck and tripped on a salt raft. Injury only… April 29 Bledisloe Wharf East 56.99m, 1184t NZ barge (BSC) During cement discharge operations the holding down bolts at the Hiab crane base sheared. April 26 South of Dunedin 60.47m NZ fishing vessel (SSM) The vessel was sheltering during strong offshore winds after dark. The vessel grounded and an estimated 2,000 litres of marine diesel was spilled, an on-site assessment did not discover any traces of the diesel. Propulsion failure… April 27 Napier Port entrance channel 188m 23,896ft Foreign SOLAS (ISM) container ship Engineers were unable to stop the main engine upon turning to port around the breakwater. Using both tugs the vessel was brought to a halt with the main engine still running ahead. The engine was eventually stopped and the manoeuvre completed using only tugs and bow thruster. Injury only… April 27 Motukopaka Island 8.95m NZ passenger (SSM) A passenger tried a "helicopter" cast technique and managed to embed a hook deeply in his finger... an ambulance was arranged to meet the returning vessel at the wharf. Grounding… April 27 Lake Whakatipu 7.9m water taxi A passenger reported that upon boarding they had been concerned that there were too many passengers - and in fact the small craft hit the bottom a few minutes later. No life jackets were seen to be available and the water taxi scraped along until deeper water where passengers were informed that the craft was overloaded and some were taken to the shore to disembark – which they eagerly did. Equipment failure… April 28 Milford Sound 25.30m NZ passenger (SSM) Down in the Sounds, the toilet on a tourism catamaran became stuck in the flush position causing the saltwater pump to run continually. Overflowing occurred mainly in the womens' toilet, and such was the flooding that the vessel began to list. Until the problem was identified sewage was pumped overboard as a relief, then shore discharge was connected. Injury only… April 28 Fergusson Wharf Port of Auckland 223m, 28,372t containership foreign SOLAS (ISM) At 10.30pm a casual employee fractured a rib. The stevedore was placing a 3-high lashing bar on as per normal procedures. He fitted the bar in place but failed to check that it was secure before he bent down to pick up the corresponding bottle screw - the unsecured bar fell onto him. Foundered… April 28 Pilot Bay, Tauranga Two unpowered recreational vessels 10 unprofessional young skippers were rescued from two separate vessels. The boys had gone out in calm weather and after stopping for icecream in Pilot's Bay found the conditions had changed. They waited, but decided to chance it, but as they rowed out the wind picked up further and their vessels began taking on water – which had reached over the gunnels by the time they were picked up by other craft in the area. www.skipper.co.nz Equipment failure… Injury only… April 30 northeast of Snares Island 65m 1386t NZ fishing vessel (SSM) While a crewperson was shooting gear, the boat rolled and a chain slid across the deck catching said crewmember about the legs, crushing and bruising them – a trip to the hospital confirmed a fracture. Near miss/close quarters… April 30 Meridian Wharf Lake Manapouri 19m NZ non-passenger workboat While berthed and preparing for scheduled afternoon departure, the workboat skipper was alerted by a uniformed individual that divers were in fact operating in the area. Our good skipper was shocked to find that the divers were operating without flags and was quick to point out another vessel that was leaving from the nearby boat ramp and posing another potential risk to the divers. Collision… April 30 Hikapu Reach 21m aquaculture NZ non-passenger (SSM) One to port and one to starboard... Vessel 1 was heading south and the other north. When looking at his radar the skipper of Vessel 1 initially mistook Vessel 2 for part of the headland. When he realised his error he turned to port to give room, but alas, at this moment Vessel 2 had turned to starboard with the same idea. Collision course. Both vessels went hard astern and portside bow of Vessel 1 hit port side of Vessel 2. Collision… May 2 Paraparamu Recreational vessel In a report made to MNZ, a concerned party would appear to have regularly observed a vessel with a badly damaged hull from the impact of driving onto trailer and with its outboard leg and prop damaged. This same beleaguered vessel is reported to have its hull welds opened up, the skipper "never does trip reports" and skippers with cargo and staff on board, the skipper "has no ticket" and transports dangerous goods e.g. LPG and petrol... operating without crew, hit two logs on two separate occasions which put the boat out of action without making reports to MNZ. "The list goes on" apparently the vessel is "an accident waiting to happen". The concerned party also requested an email response from MNZ as they have contacted them before to no avail… Injury only… May 4 Port of Timaru, Canterbury 81.70m 1913t NZ FCFV fishing (SSM) While crew was unloading, one member had a frozen 30kg box accidently dropped on his hand by a new crewman. Fatality… May 4 Whangamata Bay Lake Taupo Recreational vessel At the time of the report it was uncertain how the deceased came to be in the water. Both motors of the recreational vessel were inspected and the main engine did not start. It appears that the deceased may have been attempting to repair the engine. The report notes that fuel was not turned on, but control lever was in start position. Injury only… May 5 Wellington Harbour 200m 60,401t vehicle carrier foreign SOLAS (ISM) A pilot was injured while stepping over cargo lashings between pilot ladder and stairway where there was no clear access. The injury was initially suspected as a sprain, but was later found to be a ruptured Achilles tendon. Hit submerged object… May 5 Rakino Northland 15.6m NZ passenger (SSM) The vessel struck a log and lost its port rudder. The skipper managed to drop off the passengers before the vessel lost all steering and had to be towed to its berth. Injury only… May 6 No.2 Wharf Port of Lyttelton 25.30m Venture K NZ fishing (SSM) A fractured jaw was the result when a contractor decided not to ascend from the back shelter of the vessel via the ladder, but instead attempted to jump down to the trawl deck on the starboard side and slipped, falling face first into the tray framing. Equipment failure… May 9 Southwest of Barretts Reef Buoy 182m 17816t NZ passenger/vehicular ferry (NZISM) The 3cm radar failed mid morning while the vessel was navigating outward bound from Picton in Queen Charlotte Sound. At 1pm the 10cm radar failed when approaching Wellington Harbour entrance, however the vessel was berthed in Wellington without incident. Extreme vessel movement/injury only… May 10 south of Stewart Island 66m 2,483t NZ fishing trawler (SSM) While in the process of hauling anchor, the vessel was struck by two huge waves. The second wave caught the vessel on the starboard quarter and white water washed a crewman over the side. The crewman retrieved within 25-30 seconds. Equipment failure… May 12 No.2 Berth Napier 180m 22409t bulk carrier foreign SOLAS (ISM) On berthing, one of Napier Port's own shoreline lanyards parted when the shoreline was being tightened. Propulsion failure… May 12 outside Lyttelton Harbour Heads 177m 19998t bulk carrier foreign SOLAS (ISM) Loss of engine power reported and a new piston ring was required. Collision… May 13 East Cape Bay of Plenty 19.5m NZ fishing (SSM) The skipper contacted maritime radio advised that they had been hit by a larger vessel at around 3am, the name of the other vessel was not supplied. Rudder and other damage was reported and the vessel was drifting, but had auxiliary power for bilge pumps and radio, a tow by another fishing boat out of Tauranga had been arranged. Contact… May 13 Hokianga Harbour Northland 194m NZ passenger (SSM) During a training exercise the coastguard vessel made contact with the ramp and managed to puncture a segment of the pontoon.
