Loss of stability/injury only… 2 February Electrical failure…26 February Auckland Harbour The 7.3m NZ Passenger (NZISM) vessel was on passage from Auckland to Rakino Island. At the eastern end of Motuihe Channel while rounding the South Eastern extremity of Motutapu Island when the vessel encountered two consecutive hollow backed waves. The sole passenger was standing in the cabin egress when the bow plunged causing his knees to buckle. The second wave lifted him to roof height then he toppled and sat on his leg, breaking both tibia and fibula just above the ankle. The vessel was travelling at a reduced speed and having made previous trips to Rakino that day, the passenger had been forewarned of the conditions. Gisborne Laying to, the skipper turned on the steering unit on the 23.7m fishing vessel, there was a short circuit in the LV electrical system causing a catastrophic steering failure (auto pilot). The designated engineer suspects the wiring is aged and there was inadequate maintenance. Injury only… 3 February Tauranga A person was injured while fuelling the skiff on the 36.02m, 409.72 tonne NZ Fishing (SSM) Fishing Vessel. He slipped and fell two metres onto his shoulder. Propeller Entangled… 7 February Marsden Cove Marina The 7.09m NZ Non-passenger (SSM) research vessel was conducting a biosecurity survey in Whangarei Harbour. Wind was pushing the vessel close to the western bank, to counteract this, the skipper turned to port and applied some power to reposition into deeper water. This occurred when the crew was retrieving a benthic dredge. The starboard propeller came into contact with the dredge chain which then became entangled. It was possible to manoeuvre safely with the port engine whilst crew freed the propeller. No substantial damage to starboard propeller and engine and the survey was safely concluded. Injury only… 7 February At sea, Otago A person complained of pain on the 19.5m NZ Fishing (SSM) 90 tonne vessel. Cause of illness/pain was unknown. The injured person complained of pain just below his ribcage, within half an hour of the initial complaint the injured person was found in the fish room in immense pain. At this point the skipper phoned for medical advice. The injured person was airlifted to Dunedin Hospital. Contact with bottom… 16 February Westpark Marina Channel The NZ Passenger (SSM) Hire and Drive Charter vessel was travelling up the channel to the marina when the skipper noticed a yacht sideways across the channel 500 m ahead. The vessel was slowed and began to move to the starboard side of the yacht to ensure plenty of clearance. Due to the very low tide the amount of space was misjudged. The side of the channel was hit, the vessel was stopped to check passengers and for any leaks. The vessel floated free shortly after contact and the vessel was able to proceed to its berth. The vessel was taken out of the water for inspection. Fuel oil spill… 20 February Port Nelson The 42.5m NZ Fishing (SSM) vessel whilst bunkering spilled fuel oil via the port side vent, spilling into the harbour. The oil was contained and cleaned up in an efficient and timely manner. The Harbourmaster and Council were notified at 0045. The spill has been estimated at 26 litres. Collision… 23 February Aotea Quay,Wellington Harbour While undertaking a trip with passengers the NZ Non-passenger 32m tug was passing close to another vessel. The top of the vessel's funnel came into contact with a protruding retractable mooring platform. Person overboard…1 March Glasgow Wharf,Wellington The 136m NZ Passenger (SSM) passenger freight ferry was manoeuvring onto berth. Three passengers jumped overboard from the starboard aft boat deck of the vessel into Glasgow Basin. A verbal report was made to Beacon Hill. The police were informed accordingly. The three individuals were interviewed by police and issued trespass notices. Near miss/close quarters… 2 March Off Otonga Point, Bay of Plenty A commercial fisherman was trawling when two commercial charter vessels (with passengers aboard, including children) crossed in front of his vessel. One stopped very close in front and commenced throwing heavy sinkers at his vessel (preventing an exit from the wheelhouse). As the fisherman was engaged in trawling at the time, he advised that if his gear had fouled the other vessel's prop there may have been a collision. The skipper reported the incident to the police via 111, who advised they would pass the complaint to Tauranga Police. Contact… 2 March Auckland Viaduct The skipper of the 18.15m tourism NZ passenger (SSM) vessel was reversing out of berth when the vessel got stuck in reverse. The motor was turned off and the vessel drifted towards another vessel with fenders. This resulted in low speed contact and some superficial damage. Flooded/structural and electrical failure… 4 March West of Westport When the skipper of the NZ (SSM) trawler fishing dredger got up he checked the motor and started idling towards the SW. The vessel was doing 3.5 – 4 knots when all of a sudden the bow dipped down into the trough of a wave. A wave then crashed through all four of the front windows, throwing the skipper out of his chair and drowning all of the electronics, except the VHF and the autopilot. He boarded up the windows and steamed to Westport where Perspex was put over the windows and a GPS was located. The vessel then steamed to Nelson where the vessel is to be repaired. Person overboard/injury only… 7 March Bay of Islands The vessel was on the regular swim with the dolphins trip having left Paihia with 18 passengers. Having completed one successful swim the skipper was manoeuvring the vessel into position for a second drop with the swimmers on board, ready to go. A passenger went over the side and her leg was hit by the propeller. The skipper retrieved her from the water and first aid was administered. An ambulance was called to meet the vessel at Waitangi Wharf where the passenger was transported to Northland Base hospital. Equipment failure… 8 March Port of Auckland The 201m foreign SOLAS (ISM) container ship was making a flood tide Pelin Davison approach, and whilst backing into the berth the bow thruster overloaded the May/June 2013 Professional Skipper 79
