A lasher was injured while unlashing containers. As he was loosening the turnbuckle, the 3-high bar dropped out, the man turned to push the falling bar away to prevent it falling on him, injuring his lower back. Grounding... 8 April Motukahaua Island (Happy Jack Island) Vessel: 9.75m Recreational. Category: Yacht The vessel ran aground on the Happy Jack Islands hitting rocks. RCCNZ coordinated SAR incident as a result of EPIRB activation tasking Westpac Rescue, Coastguard vessel and a private vessel. Three people were uplifted from the island by the Coastguard vessel and returned to Auckland for medical treatment. Collision... 10 April Lake Karapiro Vessel 1: 7.3m NZ Non-Passenger (SSM) Grounding... 15 April Tauranga Harbour Vessel: 23m Non-Passenger (SSM) Category: Work boat The vessel made fast to aft end of another vessel to assist with berthing. The vessel took an unexpected sheer to starboard leaving the channel, which went undetected by the tug crew. The tug was drawn out of the channel and grounded briefly on a sandy bottom. The tug was returned to service after survey. Propulsion failure... 15 April Wellington Vessel: 150m NZ Passenger (NZISM) Category Ro-ro/vehicular ferry The vessel was proceeding from Wellington and load was increased as per normal practice. High temperature alarms were activated on the main engines. The load was reduced, simultaneously the HT Amot Valve was placed on "Full Cooling". Converters were stopped and then restarted to reinstate the propulsion motors that had tripped. After successfully completing the trials the vessel proceeded to Picton. The HT Amot Valve remained on manual control in the "Full Cooling" position. The temperature control on the main engines was achieved with the thermostats fitted to each engine. Hit by an object... Vessel 2: 12.2m Recreational Category: Rowing skiff 15 April Kaituna River Vessel: 4.3m Raft (SOP) Category: Adventure The two vessels collided when Vessel 1 was drifting while crew was discussing safety briefings. The crew of Vessel 2 initially perceived the vessel to be stationary. The collision occurred as the skipper of Vessel 1 proceeded to throttle the vessel slowly forward without realising the presence of the skiff. No one was injured. The skipper accidentally hit a rafting customer on the right forearm with a paddle. The customer was taken to A&E with a small fracture or bruising. Electrical failure... During battery installation the skipper's finger was caught between battery and battery box causing laceration and a fracture to the finger. 11 April Rangitoto Island Vessel: 213m Foreign SOLAS (ISM) Category: Container shop The vessel encountered a total blackout while transiting Rangitoto Channel. The steering was regained on emergency power, engine restarted to DS ahead. It was berthed without further incident. Fire... 12 April North of Haast Vessel: 22.6m NZ Fishing (SSM) Category: Trawler Skipper discovered smoke issuing from the air intake for the gen set; there was a fire in the auxiliary room, an explosion followed and the vessel began taking on water. Skipper started main engine to head for shore while the crew unsuccessfully attempted to extinguish fire. The fire knocked out the electronics and 12 volt VHF power system, resulting in radio failure. A flare was sent up and a fishing vessel met with the vessel and took the four crew aboard. The burning vessel heeled over and sank approx. 500m offshore south of Ship Creek viewing platform. Hit submerged object... 12 April Tarakena Bay Wellington Vessel: 8.8m Non-Passenger (SSM) On entering Tarakena Bay the vessel struck a rock, taking the skeg off the port outboard motor and damaging the fibreglass hull on the port side. There were no injuries to crew. 80 Professional Skipper July/August 2013 Injury only... 15 April West Park Marina Vessel: 14.2m NZ Passenger (SSM) Category: Passenger Near miss/close quarters... 15 April North Head Devonport Vessel: 18.2m NZ Fishing (SSM) Category: Trawler On sea trail out of Auckland, the vessel merged with three yachts coming into Auckland. One of the yachts (under sail) broached (on the starboard side) to port, across the bow. Collision was possible and evasive action was taken. The vessel was put into reverse and managed to pull away. Lifting/cargo gear failure 15 April Dunedin Fryatt Street Wharf Harbour Cold Vessel: 64m NZ Fishing (SSM) Category: Trawler During the vessel unload the clutch on the port side derrick wire worked itself loose causing the winch drum to free-spool, placing all the weight on the starboard side derrick wire. The pallet was a metre from the freezer hold floor and as one side became loose the pallet swung dropping its boxes. The flat plate of the pallet hit the wall, conveyor belt and a worker's back – with resulting bruising. Mooring line failure... 15 April Opua Vessel: 11.8m NZ Fishing (SSM) Category: Trawler Mooring line failure due to a broken mooring shackle. The boat had been checked six hours prior to this incident. A crewmember was taken to hospital by helicopter due to a medical condition. Equipment/electrical power failure... 17 April Atia Point Kaikoura Vessel: 17.7m NZ Passenger (SSM) Category: Passenger A passenger complained because passengers were not told to take up their life jackets when they were moved by crew to the outside muster station. The passengers were moved due to smoke in the main cabin issuing from a short circuit in the helm station. The crew prevented the wires from igniting further and the vessel returned to port. Contact... 18 April Voyager Maritime Museum Vessel: 13.7m NZ Passenger (NZISM) Category: Other charter Misalignment during berthing and the failure of the crew member to fend off appropriately caused the crew member's hand to become trapped between the bow strip of the moored vessel and the arriving vessel. Injury only... 21 April Firth of Thames Auckland Vessel: 8m NZ Passenger (SSM) Category: Fishing charter Due to sea sickness a charter client fainted and struck his head on the deck, losing consciousness. The skipper radioed a request to be met by an ambulance at Waikawau River boat ramp, by which time the charter client had regained consciousness and was taken to hospital for observation. Propulsion failure... 22 April Kennedy Point ramp Waiheke Island Vessel: 43.28m NZ Passenger (SSM) Category: Passenger While coming into the ramp at Kennedy Point the skipper selected reverse on all three engines. Numbers 1 and 2 selected but number 3, the starboard engine appeared to have stalled. On inspection it was noted that the main blower was not operating.
